Personal Care Program

The following services will be available to you depending on your individual Care Plan assessment:

• Assistance with bathing, showering or personal hygiene
• Assistance with eating and drinking
• Assistance with transfers including collecting and positioning of walking aids or wheelchairs
• Assistance with mobility, including the use of mobility aids
• Toileting
• Assistance with managing incontinence
• Provision of special dietary requirements
• Assistance with treatments or procedures and supervision of medications
• Individual support and treatment for recurrent confusion including cognitive impairments and behavioural disorders
