Admission Process
Firstly, our reception team can prepare
a package for you, which includes an Application Form and
a booklet from the Department of Health and Ageing which details
the “5 steps to entry into Residential Aged Care”.
If you are
considering applying for residence in Blue Hills Manor, the
following steps will need to be followed:
1. Assessment
– The Aged Care Assessment Team (ACAT) will assess you
with regards to your needs to reside in Residential Aged Care
and if eligible you will be issued with an ACCR (2624) Certificate.
2. Phone
Interview – An initial interview can
generally be conducted over the telephone. If interested in
residence at that time, an Application Form needs to be completed
and returned along with a copy of the 2624 Assessment Certificate
prior to the pre-admission interview
3. Pre-admission
Interview – On receipt of your application
and ACCR (2624) forms, arrangements will be made for you and
your family or carer to attend a pre-admission interview with
the Care Manager and a representative from the Blue
Hills Manor Administration staff. Here, the residential
care agreement and bond structure will be explained to you
and you will be given copies to take away with you to read.
4. Consultations
– Following the pre-admission interview you are encouraged
to fully discuss your plans to enter Residential Aged Care
with your family and Centrelink and/or financial advisor.
5. Final
Interview - You will be invited to a final
interview when a date of entry will be determined. The Residential
Care Agreement and Accommodation Bond should be signed and
returned to Administration at this time. These documents should
be fully executed prior to your admission.
6. Cooling Off Period -
A period of 14 days is applied within which time you may withdraw
from the Residential Care Agreement by giving notice in writing
to Blue Hills Manor. If you have paid any amounts under the
Agreement other than your care fees and charges during those
14 days these will be refunded to you.
